Participant Consent and Liability Waiver
As a participant in the Miami Hawks Wrestling Camp, I hereby agree to comply with all camp guidelines and assume all risks associated with my participation in wrestling activities, including but not limited to bodily injury, muscle strains, sprains, and other sports-related injuries.
I understand that wrestling involves inherent risks and assume all responsibility for personal injury or other losses. I acknowledge that while strong measures will be taken to prevent the spread of skin conditions such as ringworm, herpes, and cold sores, 100% prevention cannot be guaranteed. I have been informed of the assumption of risk when participating in the sport of wrestling.
As concern for COVID-19. We want to offer as much stability as possible for everyone for coming to camp. We hope that you will join us in our efforts to keep our wrestling room a safe and secure environment for wrestlers to learn, grow and prosper. I understand that there is a risk including COVID in attending camp and assume all responsibility by agreeing to this waiver. Please do not come to camp if you, your children or anyone in your close proximity have a fever, continuous cough and/or trouble breathing in the last 7 days. Please respect our decision to stay open and serve our community for those that choose to attend, as we will respect your decision to do what’s best for your family.

Medical Release Form/Consent to Treat
I authorize Miami Hawks Wrestling Corp and its designated representatives to obtain and consent to medical treatment for me in the event of an emergency. I agree to release and hold harmless Miami Hawks Wrestling Corp from any claims arising out of such treatment. I have read, understand, and agree to comply with the Medical Release Form as outlined above.
Release of Liability
In consideration of my participation in the camp, I hereby release, waive, discharge, and hold harmless Miami Hawks Wrestling Corp, its staff, volunteers, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in the camp or while on the premises. I have read, understand, and agree to comply with the Release of Liability as outlined above.
Photo/Video Release Form
I grant permission for Miami Hawks Wrestling Corp to use photographs or videos of me for promotional purposes, including but not limited to social media, website, printed materials, and advertising. I understand that I may revoke this permission in writing at any time for future uses of such media. I understand that these images will be used responsibly and with respect for the participants. I have read, understand, and agree to comply with the Photo/Video Release Form as outlined above.
Code of Conduct
I understand that participants are expected to adhere to the Miami Hawks Wrestling Camp Code of Conduct, which includes respecting others, following instructions, and maintaining a safe environment. I acknowledge that violations may result in my dismissal from the camp.
Expectations for behavior:
• Follow all instructions of camp staff and immediately consult the camp director if you are uncertain about these instructions.
• Bring all equipment that is required to participate in camp. Label all personal belongings and equipment.
• Notify a camp counselor as soon as any problem arises, big or small.
• Camp staff are available to help you at all times.
• Follow the instructions of police, fire, and other emergency personnel.
Behaviors not permitted:
• Harassment, hazing, and bullying of other campers or staff. This includes verbal, physical, and cyber.
• Using profanity or inappropriate language whether it is written or verbal.
• Leaving the camp area as designated by the camp director.
• Possession and/or consumption of alcohol, tobacco, or illegal substances.
• Tampering with fire alarms, safety, or security equipment.
• Excessive noise or horseplay.
• Violations of federal/state laws.
• Campers should review this important information prior to arriving at camp.
